Web1.1 All adult patients admitted to Medway NHS Foundation Trust will have a falls risk assessment undertaken by a registered nurse within 2 hours of admission ( except where there is an exemption in place such as day case patients and maternity) using the Falls Risk Assessment – see flowchart Falls Risk Assessment Flowchart. WebWe offer an individual treatment plan to restore a person's independence and confidence and reduce hospital admission. This may include an exercise programme to improve balance and strength, advice on how to complete everyday tasks in a way to make them easier and safer, and guidance on foot care and suitable footwear to help prevent falls.
